On this page you will find instructions for submitting required paperwork and the forms needed to keep your subcontractor file up-to-date in our office. Keeping your file current and up-to-date will expedite the payment process for work performed.
In order for us to make payment to you, we need the following items on file in our office:
- Copy of current business license OR Copy of drivers license and social security card.
- Current certificate of insurance issued directly to TMC Contractors, Inc. from your insurance company. The insurance certificate must name TMC Contractors, Inc. as the certificate holder. We require liability and Workers Comp insurance. Please ask your insurance company to fax the certificate to our office at

- A completed W-9 form. The W-9 form is the official IRS Request for Taxpayer ID number.
